To request converting to the pay per signal plan with American Digital, please complete this form.

All membership fees, receiver and special fees will remain the same with only the Base rate and Rate Per Signal being effected.

You may request a rate change at anytime under the the following plans of up to a one time (1) change request per calendar month.

A rate change must be made requested no later than the 15th of any given to become effective on the 1st of the next month.
